Please note all forms MUST be filled out COMPLETELY (there are 3 places to sign) and SUBMITTED at least 2 hours BEFORE your appointment.  Dr. Shelton takes the time to read through your paperwork and take notes BEFORE your appointment. All of this is necessary so she is prepared for meeting you!  If the paperwork is not submitted your appointment will be canceled.
